Bridges Habilitation Services, Inc offers a wide range of services to individuals with diabilities throughout their lives. We have programs to assist them and their families to lead fulfilling lives from the age of childhood to adult. We can be found working around the clock in locations of the individual's choice helping them learn essential life skills, feel a sense of personal accomplishment, participate ith their activities of choice and feel safe and valued.
Our programs are certified both by the State of Wyoming and by national accreditation organizationto provide top quality services and are recognized as leaders in our field. The purpose of this document is to describe the scope of our services offered in order to allow those we serve and those that may inquire about our services to make wise choices concerning their service provider and explore options in choosing the appropriate mix of services in their lives The Wyoming Developmental Disabilities, Wyoming Acquired Brain Injury (ABI) and (DD) Home and Community Based Waiver and the Child’s Mental Health Waiver are intended to support individuals with developmental disabilities, acquired brain injuries and children with emotional problems and their families remain in their home communities and learn skills that will assist them in becoming more independent.
Wyoming Adult DD Waiver and ABI Services Waiver
The Adult Developmental Disabilities (DD) Home and Community Based Waiver is intended to support individuals with developmental disabilities and their families so the individuals can remain in their home communities and learn skills that will assist them in becoming more independent.
The Acquired Brain Injury (ABI) Waiver is intended to support individuals with brain injury and their families so the individuals can remain in their home communities and re-learn skills that will assist them in becoming more independent.
- The Adult Waivers serve those participants who are 21 years of age or older.
- Services are provided in a variety of settings according to the wishes and desires of the participant. We offer residential services in private homes, apartments, group residences, on the job site, in community based settings and elsewhere as desired, assuring that needs are met and the health and safety of the individual is protected.
- Many of our adult services are provided 24 hours a day, 365 days a year as needed. We have a day program weekdays from 9 to 3. Here participants can associate together in a community setting and from which they can dispense to become actively involved in their community, whether it be work, play, or explore and relax as they desire.
- Services are provided to participants at times and in settings of their choice and according to the Individualized Plan of Care which is developed on the basis of the expressed desires and wishes of the participant and their support team. The frequency of services will vary from total care to scheduled training/care-giving sessions. Factors affecting the intensity of service provision include, participant desires or funding levels for which the individual qualifies.
- Funding for services is provided by the Medicaid program as administered by the Wyoming Division of Developmental Disabilities. These funds pay for the cost of staffing to provide services and associated program costs. Participants are responsible to pay for their own personal expenses such as food, housing, clothing, recreation, etc. Usually the resources for these come through payments they receive from Social Security and/or Social Security Disability benefits. Bridges does not assess any additional fees for the services rendered.
- Individuals who are part of Bridges Adult program come from a variety of sources. Sometimes they may be referred by the Division of Developmental Disabilities, but most often they seek out our program as a result of hearing about the quality of our program via word of mouth recommendation by people familiar with our reputation from across the State. Occasionally healthcare professionals will direct individual our way as well.
- Bridges provides a wide range of services directly to the participant. These provide a fairly all-inclusive menu from which they may choose the mix that best suit their desires and needs. Bridges is certified by the Wyoming DDD to provide the following services to Adults with DD and ABI:

Wyoming Child DD Services Waiver
The Child Developmental Disabilities Home and Community-Based Waiver allows for the provision of services that the regular state Medicaid plan will not cover for eligible children with developmental disabilities in Wyoming. The Child DD Waiver began in July 1992 and is administered by the Developmental Disabilities Division. Children ages birth through 20 years are eligible if they have been diagnosed with mental retardation or a related condition and meet the level of care for an ICF/MR. The goal of the waiver is to provide home and community based services that enable the child to stay in the local community rather than being institutionalized. The intent of the Waiver is to relieve pressures on families by assisting them in meeting the needs of their child with developmental disabilities.
- Services are provided in a variety of settings according to the wishes and desires of the participant. Very often the services are provided in the child’s home, but we also provide for a community based environment after school and during the summer months so the child can have the experience of interacting in a variety of settings.

Usually services are scheduled in advance so that trained personnel can be made available to meet the needs and desires of the participant and their familiesat times and in settings of their choice. We can also provide around the clock services in the home on occasion to allow family members to have a break from the demanding routines that can sometimes be a part of caring for a disabled child.

The types of services offered and the frequency of service delivery will be determined by the participant, his family and the support team and be delineated in the annual Individual Plan of Care. Often other providers will also be part of this team and help in the development and provision of services. Factors affecting the intensity of service provision include participant desires and/or funding levels for which the individual qualifies.

Children's Mental Health Waiver
Bridges is excited to announce the newest division of our organization. In 2009 we began to reach out to another group of individuals who face big challenges in their lives; children and youth with serious emotional disturbances and their families. After a great deal of preparation and training we have put together a top notch team to provide needed services to this demographic.
What is the Children's Mental Health Waiver?
The Children's Mental Health Waiver is a Department of Health Medicaid program with a limited number of funding opportunities that, by using High Fidelity Wraparound, aims to help children of Wyoming reduce their level of service needs and increase their natural supports in a relatively short amount of time.
It provides individualized services and support based on unique strengths and needs of children and youth with serious emotional disturbances and their families. It utilizes a team and goal orientated process for success.When Wraparound on the Waiver are used successfully it can:
Keep youth with serious emotional disturbances that need mental health treatment in their home communities with their parents/families involved in all aspects of their treatment thereby preventing custody relinquishment.
Strengthen families skills to support the physical, emotional, social and educational needs of their children.
Reduce, and in some cases, prevent the length of psychiatric hospital stays.
What are the qualifications for the Children's Mental Health Waiver?
* Children and youth between the ages of 4-20.
* Meet the definition of serious emotional disturbance.
* Meet at least one Medicaid criteria for inpatient psychiatric hospitalization.
* CASII composit score of 20-27 (between ages of 6-20)
* DSM Axis 1 or ICD diagnosis
* Social and emotional assessment information or ECSII (ages 4 & 5)
* Financially eligible for Medicaid based on their own resources.
Services offered on this program are:
* Family Care Coordination: services that advocate for and support youth and family utilizing the Children's Mental Health Waiver to the fullest benefit possible.
* Youth and Family Training & Support: services train youth, family members, neighbors, friends or companions specifically identified by the family care team in the Individual Service Plan that support and enhance the overall service goals for the participant. There are unlimited possibilities as to what activities are included in this service category.
* Respite: this service has been approved for the waiver. This service allows parents and other family members a respite from the daily demands of taking care of the program participant. The maximum number of respite units allowed per year is 1664, which is approximately 8 hours per week.
The costs associated with providing this service are paid for by Medicaid as administered by the Wyoming Department of Health, Division of Developmental Disabilities and no additional fees are assessed to those participating in the program. Services are administered at times and locations convenient to program participants. The services may be provided in the home, in the community or at the Bridges’ facility and are usually scheduled in advance to ensure that providers are available.
At the present time, Bridges is the principal provider of these services in our area although there are a few other independent providers. Individuals seeking to take advantage of this program are generally referred by schools, mental health professionals, or by others who are presently receiving services and wish to share the opportunity.
